Reporting to a Primary Care Clinic Director with collaborative support from an Ambulatory Medical Director, the Primary Care Ambulatory Clinic Nurse Practitioner NP will partner daily with welcoming nurses, skilled medical assistants, care coordinators, behavioral health consultants, pharmacists, and a diverse interdisciplinary team. The Primary Care Ambulatory Clinic Nurse Practitioner NP will manage a primary care panel across the lifespan, deliver same-day and chronic disease visits, champion wellness and prevention, leverage telehealth, contribute to quality improvement, and practice within a collegial culture that emphasizes shared huddles, mentorship, and strong physician-APP collaboration.
