Pay Range: $40 - $45/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities:

• Performs care management duties to assess and coordinate all aspects of medical and supporting services across the continuum of care for complex/high acuity populations with primary medical/physical health needs to promote quality, cost effective care.
• Develops a personalized care plan / service plan for long-term care members, addresses issues, and educates members and their families/caregivers on services and benefit options available to receive appropriate high-quality care.
• This position is for the McAllen/Edinburg area to complete assessments in member’s homes as well as completing documentation as per State and Company guidelines to ensure compliance.
• M-F visits 3 days out of the week, with OT possible with manager approval, documentation days 2 days out of week, traveling to members homes to complete assessments/documentation.
• Face to Face assessments completed 3 days out of the week with 2 documentation days, completing tasks, submitting all forms/assessments within turnaround times, completing outreaches

Key Requirements and Technology Experience:

• Key skills; Pediatrics, RN , Home Health
• Traveling and getting to experience member visits/gaining experience.
• Must haves: Pediatric experience
• Nice to haves: Home Health
• Disqualifiers: no experience HH
• 1-2 years in Case Management or Home Health
• Pediatric
• Customer Service Professionalism
• Required: Requires Graduate from an Accredited School or Nursing or a Bachelor's degree and 4–6 years of related experience
• RN - Registered Nurse - State Licensure and/or Compact State Licensure required or
• NP - Nurse Practitioner - Current State's Nurse Licensure required
• Resource Utilization Group (RUG) certification required.
• Evaluates the service needs of the most complex or high risk/high acuity members and recommends a plan for the best outcome
