Position Details: + Job Title: RN Case Manager + Schedule: Monday-Friday, 8 AM-5 PM + Work Model: Hybrid (50-60% remote / 40-50% travel to Sacramento hospitals) + Mileage: Reimbursed + Pay: $53.50/hr + Website: Key Responsibilities: + Coordinate care across multidisciplinary teams for quality, cost-effective outcomes + Manage complex cases from assessment through discharge planning + Mentor and serve as a subject matter expert for fellow nurses + Work telephonically and on-site to support members' health journeys + Collaborate with providers, families, and community resources to deliver patient-centered care What We're Looking For: + Strong knowledge of managed care and Medicare + Excellent communication, critical thinking, and leadership skills + Active CA RN license (Case Management Certification preferred) Job Type & Location This is a Contract to Hire position based out of Sacramento, CA. Pay and Benefits The pay range for this position is $50.00 - $55.00/hr. Eligibility requirements apply to some benefits and may depend on your job classification and length of employment.
