Now Hiring: Passionate Physical Therapist for Immediate Start

Your Role Includes

• Conducting thorough in-home physical assessments
• Creating customized, goal-oriented treatment plans
• Delivering one-on-one therapy focused on strength, mobility, and independence
• Educating patients and caregivers on exercises, safety, and care techniques
• Monitoring progress and adjusting plans as needed
• Keeping accurate and timely documentation in our EMR system (WellSky or HCHB preferred)
• Collaborating with a team of nurses, physicians, and caregivers
• Supervising Physical Therapist Assistants (PTAs)
• Ensuring compliance with all state and federal regulations

What You Bring to the Table

• Current Physical Therapist license in Virginia
• Graduate of an accredited PT program
• At least 3 years of experience (home health or SNF experience preferred)
• Strong clinical judgment and ability to tailor treatments to individual needs
• Excellent communication and organizational skills
• Proficiency with EMR documentation (WellSky or Homecare Homebase preferred)
• Oasis experience is a plus
• Valid driver’s license and reliable transportation
