Job Description

A well-established nonprofit organization in New York City is seeking an experienced Case Manager to join their team at a family shelter serving 50 families. The Case Manager will provide direct support and advocacy to families navigating the shelter system, ensuring access to needed services and progress toward permanent housing.

Key Responsibilities:

• Manage a caseload of approximately 25 families, providing individualized case management and service coordination.
• Maintain accurate documentation in the CARES database and other internal systems.
• Assist families in achieving self-sufficiency and stable housing through goal planning and connection to community resources.
• Collaborate closely with housing specialists, social workers, and employment specialists to ensure comprehensive client support.
• Participate in team meetings, trainings, and performance reviews to align with program goals and compliance requirements.

Qualifications:

• Experience: Minimum 6 months to 1 year of experience in a shelter or human services setting.
• Education: Bachelor's degree in Social Work, Human Services, Psychology, or related field preferred.
